Nortriptyline is the active metabolite of Amitriptyline and exerts its action on Norepinephrine and Dopamine. Closer monitoring is required for any patient using Nortriptyline that has a history of cardiovascular disease, stroke, glaucoma, seizures, and hyperthyroidism. Due to the risk of the discontinuation syndrome, tapering is always recommended.
