THE DOCTOR'S ROLE

It is essential that physicians assist their patients in providing compounding prescriptions during the withdrawal process. Benzodiazepines, Sleeping Pills and Antidepressants are not available in the correct dosage to effectively titrate. Therefore a compounding prescription allows a reduced dosage formulation to minimize withdrawal symptoms. Titration schedules are laid out in the POINT OF RETURN workbook and we are happy to review them with you. We have a full list of compounding pharmacists on our web site as well as a few that are familiar with our program.

UNDERSTANDING BENZODIAZEPINE WITHDRAWAL

Many patients on benzodiazepines will develop tolerance and severe withdrawal symptoms while still on their medication. These may include rebound anxiety, phobias, memory difficulties, insomnia, tremor, imbalance, sensory and motor disturbances, and gastrointestinal irritability.  Attempts at withdrawing the medication can be unbearable and unfortunately, many health care professionals do not recognize these withdrawal symptoms.  They are usually attributed to "anxiety" and the medication is increased or an additional benzodiazepine is prescribed to lessen the symptoms.  Tolerance to the new dose is achieved very rapidly and the symptoms reemerge.  The only treatment that will bring long-term success is a very slow taper of the medication under a physician's guidance.

Generally, the technique for withdrawal is to convert your patient from a short-acting benzodiazepine to diazepam, a long-acting benzodiazepine. This is ONLY recommended if a patient cannot withdraw from the medication they are currently taking. Converting to Valium in this case makes for a smoother withdrawal experience. The diazepam is then slowly tapered at one to two week intervals. Tables that outline the medication conversion and tapering schedules are available upon request.
